CMCA Application

Once you have completed a pre-approved prerequisite course, your next step in obtaining the CMCA certification is to take and pass the CMCA Examination.

The CMCA Examination is offered in two formats: computer-based or pencil and paper. Choose the method you would prefer on the CMCA application.

CMCA Application (PDF, 287 KB): Print out this form and submit it to NBC-CAM along with required testing fees. Please see the application for details of what to include.

Computer-based exam: If you choose to take the CMCA Examination via computer, dates and location information can be found by visiting www.lasergrade.com.

Paper and Pencil exam: If you choose to take the CMCA Examination via the pencil/paper format, dates and location information can be found by visiting the Schedule and Locations page.
